This evaluation form has been created as a media tool to expedite the process of requesting hardware and/or software diagnostic tools from Ultra-X PR for evaluation. Because of the large volume of requests we receive, we are unable to accommodate every request. Therefore, to properly evaluate your request, we ask that you take a few minutes to fill out the product evaluation form below.

Please review the following requirements before submitting your request. Product requests that do not meet these criteria will not be considered.

1. Reviewers must have an established affiliation with a legitimate publication. Ultra-X will not consider requests from publications that are in development or Web sites that are under construction.

2. Reviewers must be able to provide evidence of consistent product reviews on software relevant to Ultra-X's market and provide an example of a recently published article.

3. Reviewers must state the purpose of the article (for example, review, lab testing, product roundup, etc.), and indicate the expected publication date.

4. Reviewers are required to provide the assigning editor's phone number and/or e-mail address.
